Care Manager

Our services are in growing demand, so we are hiring a Care Manager!  Care Managers work one-on-one with clients to coordinate support services. Much of this work is done in the office on the phone, though occasional visits to clients at home are part of the job.  This is not a caregiver position; it’s more of a case manager.

Responsibilities: conduct in-person and/or phone interviews and annual assessments. Develop, implement, and monitor care plans for clients. WMSS Care Managers work to build strong bonds and trust with ongoing evaluations to ensure client success.

Skills & experience: LCSW or RN desirable, 4-year college degree, background in social work, nursing, or home care required. Bilingual skills in Spanish and English are highly desirable. A valid driver’s license and cell phone are required.

Salary & benefits: 32 hours per week, Hybrid/remote work schedule may be possible after 90 days. Salary: Based on experience. Benefits: Medical, 401K, paid vacation/sick days, and mileage reimbursement.
